The mentally ill may indeed be falling through the cracks of American society, along with the poor. That's the downside of an individualistic society. The government is disempowered to fix big social or psychological problems. The faith is that the competitive market will take care of it, but that works only when there's profit to be made, or when making that profit improves people's health. More profit might be made by allowing certain problems to remain unfixed.
